In some cases, labor needs to be started by induction. A variety of methods can be used to start labor. The method chosen will depend on the cervix at the time of the induction. Whether the cervix has begun to efface and dilate or not will help determine the method used.
Pitocin is a synthetic form of oxytocin, which is the hormone that naturally is secreted during labor. This drug is also used if you are already having contractions, but they are not regular or strong enough. Pitocin is administered through an IV. Often the doctor will start you off with a low dose and increase it as needed. These contractions are stronger than natural contractions. You will need to be monitored during a Pitocin induction.
Prostaglandins are hormones that cause the cervix to efface and dilate. These medications are inserted into the vagina. They are used to ripen the cervix and start contractions. These take a bit longer to work than Pitocin.
Rupturing the membranes involves inserting a plastic implement resembling a crochet needle and breaking the water. You will need to be dilated a few centimeters for this method to be effective. This method is sometimes used alone and other times used in conjunction with another method of induction. Pitocin in combination with breaking the water is a common method used to induce labor.
Stripping the membranes involves the practitioner using his finger to separate the bag of water from the side of the uterus. The cervix needs to be a little dilated for this method to work. Stripping the membranes causes hormones to be released, which brings on contractions. This method can take a few days to work. The procedure is generally done in the doctor’s office and you are then sent home.
Augmenting Labor at Home:
There are a few methods that are thought to help stimulate contractions naturally. Some people swear by these methods, while others believe they are old wives’ tales.
Sexual intercourse is one method that is believed to help get labor going. This will only work if you are showing signs of cervical changes. If you doctor sees some change in the cervix, having sex may be enough to put you over the edge into labor. Semen contains prostaglandins, which are used to induce labor.
Nipple stimulation is one way to help get contractions going. This causes oxytocin to be released. Whether this method will work or not also depends on the cervix. If it has begun to ripen, you may have success with nipple stimulation. Midwives often use this method prior to trying other methods. Don’t attempt this without asking your doctor or midwife. Nipple stimulation can cause strong contractions and your practitioner may want to monitor you during this procedure.
